Vaccination Record


While lot of applicants are waiting for the dates to open and file I-485, there are lot of things that can be done before the priority date become current, from the applicant's side.  One such is Vaccination Record.

As part of I-485, most applicants are required to file I-693 -- Report of Medical Examination and Vaccination Record.

It asks for vaccination records for:

  • MMR
  • Hib 
  • Hepatitis B
  • Varicella
  • Pneumococcal
  • Influenza
  • Rotavirus
  • Hepatitis A
  • Meningococal
  • Polio
  • Tetanus and Diphtheria Toxoids
  • Pertussis
  • Any other vaccine-preventable diseases recommended by the Advisory Committee for Immunization Practices
The form has to be filled and signed and sealed in an envelope by an authorized civil surgeon. There are expiration dates for these forms and the actual form filled by the doctor. So, it is best to wait for your date to become current and then do this form. 


But, what can be done earlier is, take this list to your primary care physician (PCP) and see what all is appropriate for your age and take those shots. Some of them might be covered by your health insurance. 

This way, when the priority date becomes current, you can request the record from PCP take it to the authorized doctor and get it signed.

  2. Should i also plan to file "I-765, Application for Employment Authorization" or that is not needed as i will any way get GC as my priority date is EB3 India Oct 8 2003

  4. If we take vaccination and medical records now, how many days they are valid irrespective of PD, I-485.

    1. It used to about a month. Now, they accept for upto year. It is preferable, you take the vaccinations now and then, fill the form and get it signed from authorized civil surgeon when your PD becomes current.

    As usual - very helpful information! Here are a few questions from my side - your response is greatly appreciated.

    My situation: Approved Labor. EB2 - Priority date (Feb 2008). Working on I-140 application while waiting for my date to become current.

    Question 1 - Are all those vaccinations required? Meaning if my PCP determines that I don't need some of these vaccinations, is that acceptable by USCIS?

    Question 2: Is it fair to assume that my PCP would be an authorized Civil Surgeon?

    1. 1. USCIS has determined the age appropriate vaccinations.
      2. In the above blog there is a list of authorized civil surgeons. More possibility is that your PCP is not.

  6. I spoke with the PCP and the doctor said most of the vaccines are not applicable for your age. I called up one of the surgeon from the website and he wants all the vaccines to be done. It's so much contradiction. Essentially I will end up forking that additional amount for vaccination records.
